Many project managers capture minutes from their team meetings which resemble an exhaustive description of the conversations that took place in the meeting.
It is a very rare project indeed that requires a verbatim account of the meeting details. It is far more effective simply to capture any actions that came out of it.
The comments and notes can be of interest, but are rarely referred to again. Far better to capture only the items that require someone to act and do something, who it is that should be doing it and a deadline.
Then make sure you circulate, so everyone has visibility of what is expected of them (and an opportunity to challenge if their understanding was different).
